Asthma : Complete Explanation About The Sign,Symptom, Cause, Treatment






Asthma is a respiratory condition marked by spasms in the bronchi of the lungs, causing difficulty in breathing.Symptoms of asthma are usually worse at night and in the early morning or in response to excercise or cold air.Different individuals react to various factors in different ways.




The symptoms of asthma
  • Wheezing
  • Coughing
  • Chest tightness
  • Shortness of breath

The symptoms of severe asthma attack :
  • Extreme shortness of breath.
  • Chest tightness.
  • A rapid pulse.
  • Sweating.
  • A bluish discoloration of the lips and fingernails.

Signs during an asthma attack

Asthma is clinically classified according to the frequency of symptoms
  • Forced expiratory volume in one second (FEV1).
  • Peak expiratory flow rate.

Asthma may be classified as atopic (extrinsic) or non-atopic (intrinsic) where atopy refers to a prediposition toward developing type 1 hypersensitivity reactions.Asthma is the result of chronic inflammation of the conducting zone of the airways especially the bronchi and bronchioles, which subsequently results in increased contractability of the surrounding smooth muscles.

The causes of Asthma
  1. Family history or combination of genetic.
  2. Environmental factors include allergens, air polution and chemicals.
  3. Smoking.
  4. Low air quality from factors such as traffic polution or high ozone levels.
  5. Exposure to indoor volatile organic compounds like formaldehyde exposure.
  6. Phthalates in certain types of PVC.
  7. Common indoor allergens such as dust mites, mold, animal dander, cockroaches, and perfumes.
  8. Certain viral respiratory infections including rhinovirus and respiratory syncytial virus.
  9. Bacterial infections of the upper respiratory tract.
  10. Psychological stress.

Asthma triggers :

1. At night
The symptoms of asthma can get worse at night because of some factors include :
  • The body clock : the levels of the subtances against inflammation to be lower at night.
  • Heartburn or Gerd : Stomach acid backing up into esophagus or sometimes into throat
2. During pregnancy
3.Taking medications : aspirin, cold medicines, and beta blockers that may cause the eyes drop.Talk to the doctor for taking medications.

Diagnosis of asthma based on
  • The pattern of symptoms.
  • Response to therapy over time including allergy testing, allergy shots, and flu shot.
  • Spirometry is the single best test for asthma.It can not be used fo children under the age of six because they are too young for spirometry.It is a breathing test that the doctor may administer.


  • Methacholine challenge involves the inhalation of increasing concentrations of a substance that causes airway narrowing in those predisposed.Negative means that a person does not have asthma and positive means that a person does not have specific for the disease.
  • Peak Flow Meters are used to measure the peak expiratory flow rate, important in both monitoring and diagnosing asthma.Peak flow readings can determine what triggers may be making asthma symptoms worse, whether asthma action plan needs to be adjusted, and whether needing emergency treatment.





Treatment

  • Bronchodilators are quick-relieve asthma medicines.They can relax the muscles around the airways of the lungs.Theophylline is a bronchodilator.It is used alone to treat mild persistent asthma and used in combination with inhaled corticosteroids to treat moderate or severe persistent asthma for taking everyday as directed from the doctor.It is taken by mouth rather than inhaled.
  • Short-Acting Beta2-Agonists (SABA) act within minutes to relieve sudden asthma symptoms.An inhaler is the type of medication delivered by a device.When inhaling a dose, the medicine quickly relaxes the muscles surrounding the airways.Ask the doctor to use the inhaler correctly.
  • Inhaled Corticosteroids treat the inflammation associated with asthma and are very similar to a chemical that the body naturally produces to combat inflammation.
  • Leukotriene Receptor Antagonists work by blocking the action of the leukotrienes, which are chemicals that can increase inflammation in the airways of the lungs.They are available in the form of pills or tablets.They belong to long-term control medicines.Leukotriene Receptor Antagonists treat mild persistent asthma.
  • Immunoglobulin E or lgE is an antibody in the immune system.lgE inhibitors prevent asthma symptoms before they occur.They are given by injection one or twice a month to people whose moderate severe persistent allergic asthma is not controlled with inhaled corticosteroids.

Many asthma medications must be taken with a device to deliver the medications directly into the lungs.

Cough : Learning Of Duration, Character, Quality, Timing, And Treatment







Cough is a sudden and often repetitively occuring reflex which helps to clear the large breathing passages from secretions, irritants, foreign particles, and microbes.A respiratory tract infection causes irregular cough.Cough is classified by its duration, character, quality, and timing.

The causes of irregular cough are triggered :
  • Air polution
  • Smoking
  • Choking
  • Heart failure
  • Asthma
  • Post-nasal drip
  • Chronic bronchitis
  • Lung tumors
  • Gastroesophageal reflux disease (GERD)
  • Medications (ACE inhibitors)

The cough reflex consists of three phases :
  1. An inhalation.
  2. A forced exhalation against a closed glottis.
  3. A violent release of the air from the lungs following opening of glottis.

The duration of cough can be divided into three :

1. Acute (of sudden onset) is present less than three weeks.It is usually determined by family history and physical exam.
The common causes of acute cough

  • Upper respiratory tract infections.They are caused by infection with a germ (virus).
  • Lower respiratory tract infections.They are caused by infection with germs (viruses, bacteria or fungi).
  • Throat clearing secondary to laryngitis or pharyngitis.
  • Post-nasal drip results from rhinitis and sinusitis.It is mucous accumulation in the back of the nose and throat leading to or giving sensation of mucus dripping downward from the back nose.

2. Subacute is present between three and eight weeks.

The common causes of subacute cough
  • Post infectious cough
  • Acute sinusitis
  • Airway hyper-responsiveness is airways  that are slow to settle down after an infection.In this case the germ has gone, but the airways are still swollen and irritable.So it causes to keep coughing.
3. Chronic lasts longer than eight weeks.For patients with chronic cough often represent more of a diagnostic challenge like individuals  with a normal examination, chest radiograph and lung function tests.
The causes of chronic cough
  • Post-nasal drip secondary to nasal or sinus disease.
  • Asthma.
  • Gastroesophageal reflux disease (GERD) may require ambulatory pH monitoring or a prolonged trial of antireflux therapy to diagnose.

A cough can be productive or non productive.It may occur only at night (nocturnal cough), or just at day, and during both night and day.

  • Non productive cough ( dry ) does not produce sputum.
The symptoms of non productive cough

- The throat feels itchy and scratchy due to the dryness.
- The throat pains due to excessive coughing.
-  It is difficult to eat.
-  Fever.

The causes of non productive cough 

- Virus infection.
- Dry or cold air.
- Air pollutants such as tobacco smoke, dust, and smog.

  • Productive cough produces phlegm or mucus (sputum).The mucus may have drained down the back of the throat from the nose or sinuses or may have come up from the lungs.It is associated with tuberculosis, bronchitis, and bacterial pneumonia.The loose productive cough is a sign of chest congestion or infection due to colds or flu.It clears the lungs and air passages of excess mucus.
Phlegm is a sticky material from mucous membranes in respirarory system.Normal phlegm is usually clear.An infection of phlegm is greenish brown.It is yellowish brown to have a cold.

The causes of productive cough

- Viral illness.
- Infections of the lungs or upper airway passages.
- Stomach acid backing up into the esophagus.
- Nasal discharge  (post-nasal drip) drains down the back of the throat.
- Smoking.


The characteristics of cough originating at various levels of respiratory tract :
  • Pharynx 
Its cause is Post-nasal drip and its characteristic  is usually persistent
  • Larynx :
Its causes are Laryngitis, tumor, whooping cough, croup.
Their characteristics are harsh, barking, painful, persistent, often associated with strider.
  • Trachea 
Its cause is Tracheitis and its characteristic is painful.
  • Bronchi
 its causes are
- Acute bronchitis and COPD : their characteristics are dry, productive and worse in morning
- Asthma : its characteristics is dry, productive and worse at night.
- Bronchial carcinoma : its characteristics are persistent and often with haemoptysis.
  • Lung parenchyma
The causes of lung parenchyma
- Tuberculosis : its characteristics are productive and often with haemoptysis.
- Pneumonia : its characteristics are dry and productive later.
- Pulmonary edema : its characteristic is often worse at night.
- Interstitial : its characteristics are dry, irritant and distressing.


The Treatment
  • Antibiotics may be required to an acute infection like pneumonia.
  • An asthma-induced cough may be treated with bronchodilators or antihistamine.
  • An Antihistamine is used for Allergic cough.
  • Cough medicines may be used for coughs associated with colds and flu.
  • The antitussives and Expectorants are two types of drug to treat cough.
  • A trial of antibiotics or inhaled corticosteroids may be tried in children  with a chronic cough in an attempt to treat protracted bachterial bronchitis or asthma respectively.
  • Physicians prefer not to suppress as a productive cough, since it aids the body in clearing respiratory system of infective agents and irritants.
  • Treatment of a cough generally involves the condition of causing it.

  • Peptic ulcer : DGL extract 4:1 : chew 300 to 400 mg for thrice a day daily.
  • Canker sores : mouthwash by mixing 1/2 teaspoon of licorice extract with 1/4 cup of water then swish, gargle and expel for four times a day daily

Caution
  • Licorice tea should never be given to infants or toddlers.
  • Women in pregnancy should not take licorice because it can increase the stillbirth risk.
  • Breastfeeding women are not recommended to consume licorice.
  • People with conditions such as heart disease, heart failure, liver disease, diabetes, kidney disease, erectile dysfunction, hypokalemia, high blood pressure, fluid retention, hormone-sensitive cancers like prostate cancer, breast cancer and ovarian cancer should not consume licorice.
  • The consumption of licorice products should not use for longer than four weeks to six weeks.
  • Licorice may not be consumed with digoxin because it can dangerously increase the toxic effect from digoxin.
  • Licorice may not be used with warfarin (coumadin) because it can decrease the levels of the blood thinner in the body or it can not work as well.

The consumption of licorice in high dosage may cause headache, fatigue, heart attack, high blood pressure, leg swelling, and pseudoaldosteronism.

People who consume licorice excessively may lead to reduce potassium levels in the blood, causing the weakness of muscle and muscle cramps.

Consumption of licorice candy, that is very high in added sugar regularly or excessively can cause health problems including obesity, heart disease, and diabetes.

People take more than 20 grams of licorice or in large dose regularly may rise blood levels of the hormone aldosterone.
